In the file linked below, OpenOffice does not respect the layering of drawing
items on top of an image.  In this specific example, on page 53 of this document
(Figure 4-7) is a frame with a caption that is set to display As Character. 
Within this frame is an image.  If you click "Send to Back" on the image, you'll
see that there are three callouts that show up.  After the document is saved and
re-opened, however, these callouts disappear, reverting to being behind the image.

I've tested this on OpenOffice 3.1 on Macintosh and OpenOffice 3.0 on Ubuntu.

Also, I ran this through the ODF Validator at tools.odftoolkit.org/odfvalidator
and recieved the following errors (though I do not know what they mean or how to
fix them within OpenOffice):

Open attached file.
Send both pictures to back. Notice the callouts.
Save the document and close it.
Open the document. Notice that the first picture is in front again.

I have tested it with DEV300m48 on WinXP. I find no reason for that behavior.

@kenpardue: It is impolite to provide a 36MB document. You should have reduced
it to those pictures, which show the error.

Ineffectual to remove the other content.  When I tried removing the text before and after some of the 
offending pictures, it started working.  Thought it was better to give you guys the raw file to look at.

Well, whan I open the file in OOo 3.1 I see no call-outs - despite what Regina
describes.
I have to send the pictures to the back to make them appear.
Then Save, Reload
-> Some of them are again behind the picture.

Also, I have noticed that it appears only when the containing frame is set to anchor "As Character."  When 
the containing text frame is set to anchor "To Character," "To Paragraph," or "To Page," the layering is 
respected.
